Part of its ConnectHealth suite of web-care tools, PatientPlanner, provides Presbyterian’s physicians with an integrated point-of-care tool for the management of chronic conditions, complex disease states and preventive services.
Another application, PopulationPlanner, provides data warehouse and connectivity functionality necessary to integrate disparate data sources for reporting purposes and connectivity to state-wide quality initiatives.
