Midwest Extended Care (MEC) hopes the partnership with SimplyWell, a provider of internet-based health risk management solutions, will provide technology, knowledge and clinical expertise to enable customers to make decisions that will lead to better health.
MEC provides clients and families with visual biofeedback that reinforces education and empowerment about the disease process and promotes independence in self-care activities. The company intends to utilize SimplyWell’s medically-based high touch solution to enhance its current offerings by integrating the care of multiple diseases and conditions and taking a coordinated, participant-centric approach focusing on individual risks and conditions.
MEC will also continue to deploy HomMed’s telehealth monitoring device, which allows Midwest nurses to provide clinical intervention to patients in the hope of preventing serious health crises and prevent hospital E.R. admissions.
