Cardiff Software Inc of Solana Beach, California has launched TeleForm for Windows, a $1,000 software package that enables users to automate the process of entering customer responses from questionnaires and forms onto a database. The program is currently under beta test at multiple US business sites, and is slated for release in January. The user designs the forms on-screen (the program supports standard, legal size and multi-page forms and includes template documents) and these are then faxed or mailed out. Users at the other end simply fax back their responses, either in the form of hand-printing or check-box entries. Once the micro receives the information, it is retrieved and loaded into the spreadsheet or database, with any mismatched responses being flagged. TeleForm runs in MS-DOS background mode with other applications in the foreground, or on a dedicated personal computer with multiple fax cards for heavy-duty users. The program supports most data formats, including Windows 3.0’s Dynamic Data Exchange, and offers a phonebook for sending to selected fax machines.
