Under the Healthy State program, Pfizer will work with the state’s Agency for Health Care Administration to target four chronic medical conditions in the Medicaid population: heart failure, diabetes, asthma and hypertension. The hospital-based disease management program will provide onsite care managers to work with chronically ill patients on a regular basis.
